Request to Prevent Disclosure of Directory Information
Ohio Dominican University designates the following student information as public or "Directory Information". Such information may be disclosed by the University without the student's prior consent under the conditions set forth in the Family Educational Rights and Privacy Act of 1974 (FERPA).

- E-mail Address
- Class Level
- Degrees Earned
- Date Degrees Earned
- Dates of Attendance
- Honors
All other information requires the written consent of the student before the information can be released to a third party. Except in situations required by federal law, Ohio Dominican University does not release listings of its students to outside organizations without the students' written consent.
This form must be received in the Office of the Registrar within the first two weeks of the semester. Students can change their permission status at any time by contacting the registrar. Directory Information will be withheld until the student releases the hold on disclosure. Ohio Dominican University assumes that by not submitting a specific request to withhold Directory Information, the student is indicating approval for the disclosure.
Students who elect to block the release of Directory Information must conduct all university business either in person with a photo ID card or by submitting a written signed request authorizing release of specified information. Such students' names will be published in the commencement program unless the students request exclusion in writing.
Please consider very carefully the consequences of any decision made by you to withhold "Directory Information", as any future requests for such information from non-institutional persons or organizations will be refused.
I realize that this will prevent ODU from releasing any information to newspapers for honors or awards; from posting my name if I am on the Dean's List; from verifying that I am a student at ODU regardless of whom is making the inquiry and regardless of the reason for the inquiry.
Ohio Dominican University will honor your request to withhold all "Directory Information", but cannot assume responsibility to contact you for subsequent permission to release the information. Regardless of the effect upon you, Ohio Dominican University assumes no liability for honoring your instructions that such information be withheld.
