Registering for Services

Ohio Dominican University is committed to providing all students with appropriate opportunities to benefit from and enjoy their ODU experience.

ODU’s Accessibility Services Coordinator is available to all students to assist in the student accommodation process. Completing the Self-Disclosure form is the first step in a discussion with the Accessibility Services Coordinator about your disability and needs for accommodations.

Upon completion of the Self-Disclosure Form, you will need to submit additional documentation. Please review the Disability Documentation Guidelines to see what may be asked of you. Review the step-by-step instructions for each type of accommodation.

  1. Complete the Self-Disclosure Form.
  2. Submit documentation to verify disability status through one of the options below:
    1. Scan and email a copy of one of the following to accessibilityservices@ohiodominican.edu:
      1. ETR/IEP
      2. 504 Plan
      3. Psychological Evaluation
      4. Medical records from a provider
    2. Send the Disability Documentation Form to your provider – a primary care doctor, psychologist, psychiatrist, etc. – to complete. The provider can then send that completed form directly to us.
  3. After receipt and review of your documentation, the ASO Coordinator will email you to schedule an intake appointment. There, you will create a customized accommodation plan for use while at ODU.

  2. Read the Emotional Support Animal Policy.
  3. Send the Emotional Support Animal Documentation Form to your provider – primary care doctor, psychologist, psychiatrist, etc. – to complete. The provider can then send the completed form directly to us.
  4. Complete theEmotional Support Animal (ESA) Application.
  5. After receipt and review of your application and documentation, the ASO Coordinator will contact you to discuss your request. If you are approved, ASO will notify Residence Life, and they may have additional documentation for you to review and sign.
